Summary Thriller

Costumed patients create a challenge for the doctors; Derek and Ben work on a new surgical technique; Richard asks a resident to perform his next surgery, angering Bailey.

Synopsis Thriller

Leah slips out of Arizona's bed the morning after they've clearly had sex. Leah leaves in a rush and says their relationship is casual, "not about cuddling in the morning." Leah says she's "easy peasy."

Meredith and Derek are still working through who takes on which responsibilities between work and the kids. Derek has to fix the butterfly wings on Zola's costume. Meredith then checks with Alex about his preparations for the Halloween party. He's gotten a keg, nothing else. Cristina overhears about the party but declines Meredith's last-minute invitation.

Stephanie is fresh out of laser-eye surgery and back to work, not taking the two days' rest her doctor recommended. Jo confides in Stephanie that Alex is still not speaking to her. Stephanie subsequently bumps into a doorway while walking through.

Hunt tells Callie that he and Emma are "no longer a thing" because Emma didn't think he was ready for a relationship, based on his hesitation about telling her that he was married to Cristina.

Arizona and Callie have each made a costume for Sofia and argue a little over which she should wear. Arizona says they should let Sofia choose, but Callie walks away and says she's "not doing this."

Webber wants one of the interns to do his surgery. Ben shows up and, after hugging Bailey, tells Webber it's a bold move to let one of the interns operate on him. Ben wants to talk about costume ideas with Bailey. She's not into it. Ben wants Halloween to be their family's thing and he says he'll figure out what they should wear.

A guy named Victor Brown comes in having been mauled by what he says was a person. He subsequently grabs Leah and bites her neck.

Bailey says Leah has to get her wound looked at and has to get tested.

Hunt shows Cristina the X-rays of the man who apparently did the mauling and they reveal that his organs are switched inside his body. He was shot twice where his heart should have been, but survived because his heart is on the other side of his chest cavity. She's excited.

Alex comes to help Jo with a young girl named Taryn who has severe abdominal pain. Her dad seems like a busy guy whose phone keeps buzzing.

Victor, the mauling victim, suddenly starts slurring his speech and apparently having a stroke.

Stephanie, meanwhile, checks on a woman whose son had to drag her to the hospital to get a wound on her leg treated. She says she suffered a bug bite of some kind about four months earlier. Stephanie pulls away the bandage and sees a massively infected wound with maggots writhing in it. Meredith tells Stephanie that she shouldn't be treating patients since she just had laser-eye surgery.

The woman with the maggots has a panic attack when the doctors start explaining her treatment. Her son explains that she's always been afraid of hospitals, but it's gotten worse since her husband died. Meredith says she has an idea and heads to the gift shop.

Shane is quickly seeing patients who are waiting, and comes to an elderly woman with a glass eye who says she wants to see her doctor, Dr Brooks. Shane is freaked out.

Shane brings Bailey back to the waiting area to see the lady, but she's gone. Bailey leaves and, as soon as she's gone, the old lady returns. She tells Shane she'll wait for Dr. Brooks.

Hunt and Cristina work on the guy with the reversed organs. Things are complicated by the fact that the organs are reversed.

Meredith brings the maggot lady an eye mask to distract her, and starts talking her through some memories of costumes her son wore as a child.

Alex is very short with Jo about her patient. She finally asks him what she can do to make it better, but he just barks out another order.

Ben walks Derek through stitching the butterfly wings and possibly gives him an idea for his brain sensors project.

Webber tells Bailey she shouldn't stand in the way of him letting one of the interns operate on him. He kicks her off the case, saying one of the things that keeps him going is feeling like he's teaching the young doctors something.

Callie tells Meredith that she's not going to the party because she can't be in the same place as Arizona.

Leah and Stephanie, both on the bench for a while, commiserate over their circumstances while Hunt and Cristina work through their surgery together, apparently rekindling some of the fun in their relationship.

Meredith goes to Webber's room to tell him she can' supervise his surgery, which he's asked her to do. He apologizes to her for having said she should have let him die and that she was the wrong choice for making his medical decisions.

Alex realizes hat the young girl he's treating is not producing enough adrenaline, which is causing her stomach aches. When he and Jo get to her bed, they find she's missing. Jo had told her earlier that pediatric patients get to trick or treat around the hospital, so she's headed off on her own. Alex says her lack of adrenaline could cause her to go into shock if she is actually frightened by something. Meanwhile, Shane tells Stephanie about the old lady with the glass eye.

Alex's patient, Taryn, is wandering the halls alone when she crosses paths with the old lady with the glass eye, who is stumbling around and reaches out for the girl, who collapses.

Alex comes onto the scene and scoops up his patient. He also has the old lady sent to the E.R.

Callie offers to stay with the maggot lady after her surgery, but the woman tells Callie that she has to soak up these moments with her child.

Jo tries to talk Taryn's dad through his daughter's diagnosis and ensures him he's a good dad, despite what Alex said.

The "zombie's" victim wakes up and thinks he's been quarantined. April has to snap the guy out of his paranoia and explains the whole situation to him.

Bailey gets home to find her house completely decorated with smoke and flashing lights. Ben has arranged it all and given Tucker a Halloween that has made him very happy. Bailey thanks Ben profusely. He tells her he quit his residency.

Arizona walks into the locker room and finds Leah, who explains that she was bit and is waiting for the guy's HIV results. Leah says she didn't say anything about it because she's trying to be "easy peasy." Arizona tells Leah about her first needle stick, when she was treating a patient who had AIDS. She says her whole body went cold. She tells Leah this isn't the time to be "easy peasy."

Shane checks on the old lady with the glass eye and finally tells her that Heather died three months ago. The woman cries a bit and Shane says he hasn't found her in the system, realizing that Heather was treating her under the table and giving her inhalers for her asthma. The woman says she didn't want Heather to get into any trouble. Shane tells her that if she needs help again, she can come see him.

"She was a good doctor," the woman says. "Are you as good as her?"

"No," Shane says. "But I will be."

Meredith is walking Jo through Webber's surgery, and Jo wants to talk about Alex. Meredith doesn't want many details but tells Jo that Alex needs her to stick with him when he's vulnerable.

Bailey is upset that Ben is making their relationship unstable. They argue over it while kids knock on their door for candy. Bailey tells Ben she's "disappointed" in him for giving up on surgery after having left for his residency just a week after their wedding.

Hunt calls Emma and leaves her a message about how he "can't do complicated" right now, and stammers through a message. Emma calls back right away.

Meredith, trying to leave, sneaks out while Jo is explaining Webber's procedure to him.

Leah is celebrating at the bar after her tests have come back clean. Stephanie is bummed because she still can't see. Jackson shows up and says he's taking Stephanie home to rest her eyes and do nothing.

At the party, Callie and Arizona are both there and Sofia is wearing the princess consume. Callie tells Meredith that she realized she doesn't want to cut in half the number of Halloweens she gets to experience with Sofia, and that this is the first thing she and Arizona have agreed on in months.

Cristina shows up with cupcakes -- which Meredith had previously asked her to bring if she was going to come to the party -- but after seeing everyone so focused on the kids, and without anyone having noticed she is there, she leaves the cupcakes on the entry table and leaves.

Cristina ends up at the bar and runs into Shane, who buys her a drink.

Jo comes to Alex's door, dressed like Tinkerbell, and apologizes for having made "one bad call" and injecting herself into his situation with his dad. He opens the door and lets her in.
